Transitions Birth Center (“TBC,” “we,” “us”) operates the website transitionsjax.com. This policy explains how we handle information collected through this website.

When you complete the enrollment form, we collect your name, email address, phone number, expected due date, and payment information necessary to process your Facility Fee Agreement. We do not collect or store personal health information through this website.
We may also collect standard web analytics data (pages visited, browser type, approximate geographic region) through anonymized analytics tools. This data is not linked to your identity.
How We Use Your Information
Information collected through enrollment is used solely to process your Facility Fee Agreement, communicate with you about your birth center reservation, and coordinate with your midwife as needed.
We do not sell, rent, or share your personal information with third parties for marketing purposes.
Data Security
We use industry-standard encryption (TLS/SSL) for data transmission. Payment information is processed through secure, PCI-compliant payment processors and is not stored on our servers.

Who We Are
Sterling Collective, LLC d/b/a Transitions Birth Center (“TBC”) is a licensed birth center facility in Jacksonville, Florida. This Notice applies to health information created or received by TBC in connection with your use of our facility.
Note: Clinical care at TBC is provided by independently credentialed midwives, not by TBC as an employer. Your midwife's practice (e.g., Red Moon Midwifery) maintains separate health records and a separate Notice of Privacy Practices.
How TBC May Use and Disclose Your Health Information
Treatment. We may use or share your information to coordinate your care with your midwife, hospital (in the event of a transfer), or other treating providers.
Facility operations. We may use your information for internal quality review, licensing requirements, and regulatory compliance.
As required by law. We will disclose your information when required to do so by federal or state law, including mandatory reporting obligations.
Emergencies. In a medical emergency, we may share information necessary to protect your health and safety or that of your newborn.
We will not use or disclose your health information for any other purpose without your written authorization.
Your Rights
You have the right to:
- Request a copy of your facility records
- Request corrections to your facility records
- Request restrictions on how we use or disclose your information
- Request confidential communications regarding your care
- Receive a list of disclosures we have made
- Receive a paper copy of this Notice

Complaints
If you believe your privacy rights have been violated, you may file a complaint with TBC or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
